ELIGIBILITY
Who can become a member?
The Carriage House is open to any adult (18 and older) who has been diagnosed with one or more of the following mental health conditions. Membership is completely free — there are no dues, no fees, and no time limits.

Come for a tour
Stop by The Carriage House for a tour. A staff member and current member will show you around, explain everything we offer, and answer any questions.

Confirm your diagnosis

We'll verify your diagnosis. If you're with Parkview or Park Center we can do this same day. Otherwise your doctor can complete a simple referral form.

Complete your new member packet with a staff member's help and receive your Member ID card. You're officially part of The Carriage House family!
